
Nursing Home Visit
There are two types of eye test applications in a nursing home:

Private Appointment
Patients that do not have a valid medical card or do not wish to wait on the HSE Medical Card Scheme can avail of a private service. Please call us on 0858772660 and we will arrange an appointment.

The cost of this service is €89 for eye test only. Our Optometrist will have a wide selection of frames to choose from on the day.

Medical Card Appointment
This appointment type is only available to residents with a valid medical card.
​
When a nursing home resident is looking to get an eye examination under the Medical Card Scheme, the following procedure has been laid out by the HSE. HSE approval must be obtained prior to booking an appointment.

STEPS FOR MEDICAL CARD PATIENT
Step 1: Ring your local health office and obtain a Blue LOA/3 Form & Form B.
Step 2: Fill out both forms as required:
Blue LOA/3 Form - one form per resident, signed by a nurse twice on the front (this form cannot be photocopied)
Form B - allows up to 15 residents per form, signed once by a nurse on the back

Step 3: Return the completed forms to the local health office

Step 4: The local health office will either approve or refuse the application and then return it to the applicant i.e the nursing home
​
Step 5: Ring Opticall when you have received the approved application forms from the HSE
​
Step 6: Opticall will arrange an appointment to carry out the eye test and dispense glasses if necessary
​
Note that these forms expire within 90 days. After this time the whole procedure must start again.

Authorisation must be obtained from the HSE before an eye test can be carried out under the Medical Card - i.e. The LOA/3 Form & Form B must be stamped by the HSE and be in date.
